You must also be residing within the relevant boundary for a minimum of 3 months. The Freemen’s Common Health Centre NHS Services Registration We recommend that you register with The Freemen’s Common Health Centre as the University purchases a number of private services from them on your behalf.  These are not available to you unless you are a registered patient at the Health Centre.  Please complete the registration form paying particular attention to the Leicester Address, Previous GP and Previous UK address.  If you have never had a GP in this country please complete the date you arrived in the UK.  For Self-Catered Halls it is essential to wait until your room number is known, for all other addresses please put your full postal address.  Previous health information is also required to assist in any emergency care that you might need before a full history can be obtained. NHS numbers are usefull if you can find it but do not worry if it isn't available. DO NOT WAIT UNTIL YOU NEED TO BE SEEN as your records can take some time to arrive. CLICK HERE to obtain an application form. Please complete the form as fully as possible before printing (handwritten additions/alterations will be accepted). Then please do one of the following:- Post the form to Freemen’s Common Health Centre, Freemen's Cottages, 161Welford Road, Leicester. LE2 6BF (This is the quickest option, especially if you can get the form to them before the start of term) Bring the form with you when you come to Leicester and take it into the health centre during normal opening hours. Staff from the Health Centre will be available on the afternoon of Sunday, 28th September to collect the application forms and answer any questions. This option can take up to 28 days to process your application. 13:30 – 16:30 at Digby Hall, Beaumont Hall, Gilbert Murray Hall, Stamford Hall or 11:00 until 16:00 at New Hall
